They appear when your brain starts to break down, and they can hurt you when it gets bad enough, so they're things that aren't really there cuz you're seeing them when you're insane right? Well, no. They're real. They pop out of the nightmare lights regardless of how insane you are. The people who watched Maxwell's show could see them, They were recorded on film during Maxwell's final performance. You aren't hallucinating, you're seeing what's really there. They're always there, around you, watching. It's only when you go 'insane' that you can see what's real. The nightmare amulet doesn't make you insane, it lets you see the truth. Makes sense to call it sanity when viewed from the perspective of the game, which has late 19th century ladies and gentlemen who come to learn that thy are in a world filled with magic... Magic that depletes you and exposes you to the 'real' world under the surface of the one we live in. On the surface you have to really do some out of the ordinary stuff (like use a magic stick and gem that freezes things, or hang out with a giant one eyed deer) and it depletes something in you that brings this dark underworld to the surface, to horrifying effect.The ones who made the ruins went too far/dug too deep/did too much and that world is a permanent reality there due to their machinations. There's also a pretty high chance that the sanity monsters are "Them", which seems logical at this point from Maxwell's descriptions of them and how they appear a lot in the puzzles. Maybe 'insanity' brings you closer to the hallucinations world, allowing you to interact with it. The obelisks are made of two different materials, one which is more in your world (high sanity = up), and one which is more in theirs (low sanity = up). So when you're sane the one from your world seems real, and the other doesn't do much except appear to be there. And when you're low on sanity, it reverses. It seems rather than a gradual progression these materials resist being in a partial state, but once a threshold is reached they suddenly snap into their new states, like pushing on a stuck door until it suddenly opens. But I can't shoot down one theory without offering my own, so here's what I got. The Obelisks, whatever they're made out of, were created by the civilization that once inhabited the ruins as housing for the Hallucinations that doubled as a good indicator of when the things took physical forms. The apparaitions that fill up an Obelisk when you're either going insane or becoming sane again? They reside in the Obelisks and give it its length, obviously.
