After another batch of thorough testing, i'm pleased to bring you;

The Ultimate Guide to - STRESS!

First, let's start with the buffs and debuffs, and their real effects:

What doesn't effect stress?

Heat. Although the wiki, and other sources suggest otherwise, it doesn't.
Hunger. Although the wiki, and other sources suggest otherwise, it doesn't.
Lack of Oxygen. Although the wiki, and other sources suggest otherwise, it doesn't.
Hygiene.
Illness.

Something to note, is that although decor does effect stress, it takes a fair amount of negative decor to actually give you the debuff.
Often times you can only reach this value with dead bodies, and clutter.
Your "Decor" Stat (which is different to the decor value of the area) can sit at 0 without getting the debuff.
A dupe with 30 decor expectation, will happily sit in an area with a decor rating of -135, and a decor stat of 0, without getting a debuff, as an example.

What happens if my dupes are stressed?

Once your dupes fall into the stressed state (stress above 60%), they have a chance of enacting their stress response.
This will either be "Destructive" or "Vomiter". You can see which response your dupe has in "Stats - Traits".
Although the tooltip says your dupe will need to reach 100% stress to have a "Nervous Breakdown", this is not true.
Once your dupe enters the "stressed" state (above 60% stress), they have a random chance of enacting their stress response, which works out to an average of once per cycle. This is the same no matter if your dupe is at 61% or 100% stress.

A destructive dupe will break a machine, but wont break anything that has been flooded.
A vomiter dupe will vomit 20kg of contaminated water.

Notes :
The sleeping buff is not applied when narcoleptic dupes fall asleep.
The sleeping buff IS applied to dupes using a massage table.
The Making a Mess debuff is not applied when vomiting.
The New Friend buff is not applied for debug spawned dupes, only those that come from the Printing Pod.

As for how best to counter it? Either sleeping on the floor, if you don't mind the -1 athletics debuff for sore back, or when it comes to bed time,  put them on a massage bed. It counts as sleep, and they can't get the interrupted sleep debuff. Plus they also get the "relaxing" stress debuff too.

I'll try it, but the main problem is they get up to pee/eat every night, and they wind up with the debuff every day.

Once you get mechanical toilets, you can deconstruct the outhouses.  Dupes won't interrupt sleep to go pee/poo with mechanical toilets, but will with outhouses.  Also, as suggested, Isolate the loud-sleepers by a few tiles.

I just noticed this one today too.  I'm pretty sure it is caused by Trench Stench since I had 2 dupes get this status while standing next to one that had the disease. 

Spoons, great (series) of guides, they are very helpful.

To expand on Decor and the stress relationship, perhaps you could add the following:

Decor Expectation ("Expectation" is a function of the Dupe Level, Traits, etc. etc.

The Decor Need score ("Need") fluctuates between 0.00 and 100.00 based on the Decor score of the grid square where the Dupes feet are each second ("Total Decor") and the current Need score. 

  • When Total Decor > Need, Need changes by +0.25/s (tooltip) to the lower of the Total Decor score or 100.00 (e.g. a dupe standing on a Total Decor score on 150 will max out at 100.00, and a dupe standing on a Total Decor 75 will max out at 75).
  • When Total Decor < Need, Need changes by -0.17/s (tooltip) to the higher of the Total Decor score or 0.00 (e.g. a dupe standing on a Total Decor score on -50 will drop to 0.00, and a dupe standing on a Total Decor 25 will drop to 25).
  • Where it actually stops is often +/- 0.10 from the Total Score.

Duplicants gain the Decor stress buff/debuff based on the difference between their Need and their Expectation.  

  • Where Need > Expectation, the stress buff is applied.
  • Where Need < Expectation, the stress debuff is applied (and status Drab Decor).

Unfortunately, that's not how it actually works. That's how it should work though.
Dupes can more often that not be sitting on 0 decor (the stat) but still not get the debuff, and same goes the other way.
The rest is true, and i'll probably add that to the guide, but until i can narrow down how it actually works further, i'll have to do more testing.
I tested it a little, and found that even with dupes with a high decor expectation, and having been sitting on 0 decor (stat) for a full cycle, and also being in the vicinity of a heavily negative decor value, often still required multiple dead and rotting bodies to get the debuff, while some didn't.
I believe that dupe level may also play a part.

I didn't know if they was on a massage table during the night it counted as sleeping. I always stopped it, so they could go to bed. Well that's good to know!
