There is my "Diseases Expanded" that I aim to be similar difficulty level with vanilla ONI at first, but with difficulty increasing as the game progesses. I'm ofc not objective, but I like it  And yes, you will need hospitals there.

There is also "Diseases Restored" by Skyrunner, it also gets updated every now and then, though sometimes you must wait a bit longer for the update. This one is fameous for killing your base if you don't have hospitals in time
