Caves are a problem specifically, for many reasons including Nightmare Clock, players being separated ie. some in caves, some not, which would require multiple "worlds" running simultaneously, cave lighting and all sorts of things. Lots of technicalities to be worked out, and even if the idea is sound, it's still possible that it doesn't really flow well in a multiplayer network environment.
