Make Thulecite Medallion works in the surface.

The medallion works because Nightmare Fuel is everywhere in the caves and the ruins, like they literally came through the ancient gateway in the ruins. So logically the medallion would sense nightmare cycles in the caves because that's where all the shadows are. Shadows aren't present on the surface nor the fuel.

What if we use Pure Horror in the medalion to make it works. Making it more sensitive to nightmare cycles in the underground.

Cross shard communication is certainly possible, otherwise how does ant lion boulders spawn on you?

I recall a mod allowing merm king to work between shards, which means it is very possible.

It's a neat suggestion, certainly would be very useful so you don't endure loading screens just to maybe see if there's one, also would give the item like a purpose for once.

It can be done but pretty pointless

I think if it detected shadow rifts it would be pretty good to have. Lunar rifts are on the more commonly inhabited shard, so they don't really need something like this.

This would be great. We could have a different item to that. A dreadstone medallion maybe. We would interact with the new content a lot more.
