Why don't you keep all levels on surface (caves and ruins), like 3 distant islands. But only current level (for example ruins) should be available for users, when they check map. And sinkholes should act like wormholes. And with some fine art on map and other details you should create an easy illusion that there is actually something down there and not on surface. I think is still better to have 1 server instead of 3 servers (with players on each of them), which is very tricky when comes to optimization.
