As simple as this:

I equip an icon I own, enter a game and in game it acts as if I didn't equip it.

Been having the same problem and been reporting it over and over. If you encounter this problem a solution is to change the profile picture two times: pick one, quit back to the game menu, and redo, it will apply properly the first picked and ignore the second when you go in game.
