Steps to Reproduce
Load a game whit a rocket in mission.
Go to bugs

It looks like what happened here is that a module part-way up the rocket was destroyed. Everything "below" that point was disconnected from the command module and reappeared on the asteroid, and everything above it is still in space.

We're still looking in to why modules are being destroyed.
