I really believe the fog effect currently restricted to the mass graveyard section in the world could really be made more mechanically functional for other purposes in the game. I mean there are just so many facets that could be explored to add further depth and challenge to the game just from something like this. Off the top of my head it could be:

 

- A new random weather effect that obscures vision (I've suggested Shipwrecked's heavier ashen fog before, but the regular fog will work too).

This could be a blanket (multi-layered) effect reducing how clearly you see things, appear in patches that block out vision entirely until you walk through it, or even force the camera to zoom in a certain amount, to throw a few ideas out there. In addition (or alternatively if you prefer), it would also reduce the effectiveness of light sources, reducing the ring of vision you get from the varying sources currently in game.

 

- Something that appears randomly in the world at different intervals and drains sanity when you stand in it (written off as the character getting nervous).

This would stack with other sanity drains like dusk / night cycle and so on... ideally though for the more challenge-oriented people that frequent the forum, I think a multiplier might be worth looking into. (Soaked in the rain + dusk/night + fog = Deerclops level sanity drain)

Another alternative could be in the form of losing some sanity when you take damage in it (any damage, including starvation)

 

There is a lot of untapped potential here I feel, and the best part is that the tools already exist to make it happen - there's already a cloudy effect when you zoom too far out that could be plucked and applied to supplement these ideas to save on artistic implementation, the characters already have examination quotes for when they can't see (the ones when you try to inspect something in pitch blackness), most of the concepts just need the coding to bring to life.
