More dups than certain atmo suit checkpoint can handle try to use the same checkpoint at the same time. When there are not enough suits the ones who didnt get a suit for that checkpoint just continue through the checkpoint without a suit and go die (they still do the putting on a suit animation, just no suit). Example: my base has 3 entrances with each having 8 atmo suits per checkpoint (for a total of 24 prepped atmo suits, 8 at each entrance). When my 12 day dups wake up in the morning, all 12 run to checkpoint 1, take all 8 suits and 4 of them go right out the checkpoint without any suits on leaving 2 other completely full checkpoints. It has made the game almost unplayable as each shift I have to just camp at my base entrance to catch anyone who doesnt have a suit and manually move them back into the base to go through the checkpoint again.
have a base with a centralized atmo dock system (dups suit up when leaving the main living quarters). Have 3 doors they can exit the living quarters each with a checkpoint and 8 docks at each checkpoints. have 18 total dups. 12 dupes on day shift, 6 dups on night shift. Day shift wakes up from bedtime and all try to leave the base at the same time. This bug occurs any time more than 2-3 dups try to go out the same exit at the same time. This bug only started after the latest QOL release.

A developer has marked this issue as fixed. This means that the issue has been addressed in the current development build and will likely be in the next update.
