Pipes become blocked when they shouldn't be blocked.
This happens across all of my saves at one point or another.
In this screenshot the lavatory pipe bridge is not working, the pump says that the pipe is blocked.

Start a new game Build a pump system Eventually, a pipe becomes blocked without a reason for being blocked.
