This has happened to me a couple times while messing around in debug. If a dupe is picking up a dead dupe to deliver to a tasteful memorial, and another dupe walks by, they'll pick up that one too, and deliver them both to the memorial. Memorial displays the name of the (formerly) living dupe.
Meep was delivering a Mi-Ma, and Bubbles walked by, he picked her up too.

1. Have a dupe picking up a dead dupe 2. Have another live dupe very close by

A developer has marked this issue as fixed. This means that the issue has been addressed in the current development build and will likely be in the next update.
