It took me a while to realize that it wasn't a new ambient sound...
Lurking Nightmare has a unique water-like sound effect that doesn't get louder as the character moves away from it, like other sound effects in the game, but plays at the same volume wherever and whenever.
Therefore, during the nightmare phase of the Nightmare loop, the player will often hear this sound in the cave, and the Lurking Nightmare may be ten meters away from the screen on the opposite side of the cliff.
