I figure in addition to possibly having some sort of spectral pirate come after you when your sanity is extremely low, the familiar Crawling Horrors and Terrorbeaks should still be there, but they should have amusing nautical hats. Even if this isn't going to work with the official game, it'd make a wonderful mod. I imagine a Terrorbeak at sea wearing inflatable armbands and a snorkel backstroking towards a terrified Wilson as he flees on his row-boat.
