I give up! (no I don't, please help me haha) Custom hand item

I have been trying at this for an extremely long time, all day in fact, and have just hit a wall.
Tutorials attempted to be followed, characters edited. I'm lost. 

I used the hand items template and could load it into the game just fine! My axe is there and looks great.
My character mod works just fine! still a WIP but he's functional and looking good! 

When I try to add the weapon onto my character that's where everything breaks and it just won't work and I'm lost as to why. 

I don't know what is needed to figure this out so here's the whole thing. Please me know if there's anything else needed
