Item's equipped image showing up entirely wrong

I tried making my own hat, but this is what happened:

It shows up as the ground animation when equipped.

And when turning to the side or back, it disappears entirely.

 

I have no clue what I'm doing wrong... I tried to copy this hat example, but it didn't work.

Edit: I was naming files incorrectly. Oops...
