How the heck did Banhi get in there?

I had a duplicant die while I was mining out an area of the map and doing other things (washing dishes, etc) at the same time, so not paying full attention to the game. I wanted to see how she died, so I reloaded the game, only to see her somehow stuck in a place she could not possibly have gotten into. I'm pretty sure the place she's in was already mined out, but upon reloading, I don't see it even partially mined out.

Steps to Reproduce

Not sure how to reproduce this one. Mine out an area, and reload if a duplicant dies?

Just to be sure it wasn't an issue with sand falling and trapping her in, I reloaded the game a few cycles back. This is the way that area looked a few cycles back.
