This has happened many times to me, but I don't have a screenshot handy at the moment. Will add one when it next happens.
Essentially, whenever I'm building powerlines or liquid pipes (I think air pipes too), sometimes when dragging the outlines, I hear a "plop" but it still keeps drawing the outlines when I move my mouse. Then after I release the mouse button, the outlines are drawn but they have no underlying build instructions, e.g. the pipes don't get built. When I re-draw them again, they do get built, but the outlines remains visible on the screen forever until I reload the game.
Cancelling over them has no effect. They're just a ghost outline that you can't interact with anymore.
