I'm wholy not sure how to reproduce this, however, I just made a new colony, and when the game started, the starting area with the dupes, dupemaker and box was filled with stone/earth/dirt instead of being dug out as normal. Causing my 3 dupes and everything else to be entombed, giving me an insta game over.
