There's an annoying issue with "supplying self" errand. When a duplicant is operating a machine and then need to gather material to keep working, the duplicant will gather said material, put it in the machine requiring it, and then will leave to do something else.
For example, researcher will be studying at the computer. The computer will run out of water. The researcher will go get water, put said water in the computer and then just leave to do something else.
Same goes for "operators". The operator will be at the rock crusher, crushing fossils. Then he will go get another batch of fossil to crush, put them in the rock crusher, and then just leave.
If the duplicant is interrupted with the move command, the duplicant will then properly resume working with the newly supplied machine.
My understanding of the issue is that, after delivering the material to continue the task, the dupe will choose it's next task before the machine is refreshed and flagged as being "available", though I might be wrong in that regard.
Have a duplicant self supply to a machine it was operating. It seems to be pretty systematic. Shouldn't be too hard to reproduce if that's not in some ways hardware related.
