When choosing dupes with Iron Gut and using Mush Bars, I've continually seen only those with Iron Gut be impacted with diarrhea. No other dupes pick up the diarrhea condition at any point unless they have Iron Gut as a trait. I admit, my RNG may hate me, but it is extremely suspect over the multiple games played per the included steps to reproduce.
Steps to Reproduce
Game 1) Single Dupe with Iron Gut, only Dupe to contract diarrhea. Game 2) As above Game 3) Testing Iron Gut, 3 starting Dupes with Iron Gut. All three contract diarrhea. Game 4) No Dupes with Iron Gut, no diarrhea on any Dupe. Game 5) Single Dupe with Iron Gut, only Dupe to contract diarrhea. Game 6) Two Dupes with Iron Gut, both contract diarrhea. Game 7) Current Game, 1 Dupe with Iron Gut to start, contracts diarrhea. Second Dupe with Iron Gut post-start, no diarrhea at this time.
