Not sure if this is specific to the DLC, but sometimes hatches around my base will get stuck burrowed and unreachable until they die of old age. Sometimes they'll be in the air and sometimes they'll be wedged between tiles. Sometimes, a completely free hatch will suddenly get stuck after being wrangled.
