The Scrappy Scavangers introduced a super iffy glitch with switching items: it's like the game double-taps items from the equipment slot when equiping or switching them, resulting in both getting removed or switched up. It happens around a third of the times when switching items.
Reseting the controls did not resolve the issue (it did however resolve the bug where it was not possible to click on items while moving that was introduced in said update).
No mods were ever installed on this client.

Seems to have been an issue with the local machine after all. On my main system the game behaves normally. The bug ticket can be closed.
Have one item in the equipment slot (head slot, armor slot or hand slot) and switch to another item in your inventory using the hotkeys. There is a big chance that both items get unequipped immediately.
