Despite me playing Island Adventures on this, this always happens despite you got mods on or not. Only fix is by going into caves and out or using a command to "relog" for a quick bit.
Steps to Reproduce
Discover most of the map and or age the world a bit more then turn off the server. Turn it on again and you'll get this randomly generating fog of war.
