Steps to Reproduce
Scrappy werepig can become multi headed if you partially reveal him, leave for some time, and then come back to work on freeing him again.
Edit: apparently this isn't only a visual bug, they are actually separate entities.
Edit Edit: The second one is actually not summonable, and disappears if you relog.
