You can throw dumpbell on nightmare creature before it become angry (with more than 15% sanity) and actually deal DMG to them. It shouldn't be able)
Steps to Reproduce
1. Make a dumpbell
2. Lower your sanity nearly 50%
3. Throw dumpbell on inactive shadow creature
4. Realise that it's take DMG and become angry
