Creating Custom Items

Hello,

As you may be aware by the title I want to figure out how to make custom

Items this includes tools or food

I would like to know for example how to make

Coconut - + = + =

Poison Spear -  +  = 

I realise that this is very difficult

If someone can help me with this that would be very useful

I have very little knowlage about this kind of stuff

PS: I do not own DS SW so I don't have these files I hope the images make it clear
