When your world are generated, they are make on 2 islands, 1 are the standards one, while the hoter are a spécial "cave" and smaller island. The only way to get this spécial "cave" island is to going trough a wormhole, maybe a special wormhole (maybe a darker one) wich more or less sanity drain. This special island will have a sanity drain zone and with the feature we found in caves. It's just a idea, as a temporary measure while you try to implement a cave? I'm not thinking as a definitive feature, just for a moment while the devs are working on adding cave. I'm bad at english, sorry.
