Issue Title: Losing all max health with effigies gives char select, with hearts makes you die permanently

Issue Description: If you keep making effigies/getting revived with hearts, then kill yourself by one of those methods, different effects occur.

Steps to Reproduce: Dealing the last blow by placing a meat effigy brings you to character select, letting you come back with a fresh character with full max health.

Being revived with a telltale heart which would cause you to go to zero max health causes you to be stuck in death, but no longer a ghost. This state is a little glitchy, as it appears you still have hunger and sanity (Willow can keep making low-sanity fires, and if you starve you will spam death sounds and death messages on every tick of damage).
