why is Local CO-OP sooo difficult?

The  game has Together in the title yet CO-OP is quite difficult.

For PC, I have not been able to get multiplayer to work.

For Xbox, I was able to get into local CO-OP by following several steps.  (Launch with user with Xbox gold and then add a second Xbox live only profile from the title menu).

 

Can we get local CO-OP working on PC?

If we can get a PAUSE button with this I will buy all the DLC for all 5 copies of the game on all of our family accounts and convince my Steam friends to get into the game.
