Duplicants keep using the Metal Refinery after completing a recipe; the task bar doesn't show, but the building is consuming power and generates heat (and wastes time). They "update" after a while, and begin working on the next batch.
I found that disconnecting power or giving a "disable machine" command while in the "stuck" state makes the dupe immediately leave the machine as it's inoperable, but the Metal Refinery then plays an animation of the completed product rolling out of it on the conveyor belt. Reconnecting power or canceling the disable command makes the duplicant begin the next recipe immediately, without the aforementioned "stuck" state.
The issue presents itself at all game speeds.
Given that a building update (losing power or disabling it) solves the issue by brute force, I suspect that this "recipe completed" animation might be missing/bugged, and that's causing the issue.
I have made an entirely new world without any mods enabled to test if it's a vanilla issue, and the issue still presented itself. I used Sandbox mode to quickly test it after generating the world.

"Stuck" state:
1. Set up Metal Refinery
2. Queue some recipes (the issue might take a few batches to present itself)
3. There will be a "stuck" animation/phase between batches where the duplicant will keep "working", but the task bar doesn't show up and the building still consumes power and generates heat.

"Batch complete" animation:
1. Get to the "stuck" state
2. Disconnect power (snip the wires)
3. The Metal Refinery will then play an animation of the completed product rolling out of it and falling to the ground.
