The Supply Teleporter graphics appear to be swapped.
The Supply Teleporter Input currently has arrows pointing up (away from the machine), suggesting something should come out of it (when in fact it's an input, and you're supposed to send things into it).
The Supply Teleporter Output currently has arrows pointing down (towards the machine), suggesting you're supposed to feed something into it (when in fact it's an output, and things come out of it).
They function correctly (the input receives and the output, er, outputs), but it looks like someone swapped the graphics.
1. Explore map until you find the Supply Teleporter Input and Output.
2. Use eyes to look at the arrow direction (more obvious during operation, because they move).
