It happens A LOT, 50% of all chores, I've ever witnessed with Rock Crusher and Microbe Musher, but it might happen with other stuff, like the Manual Generator, but that one is rare.
Basically, after a task, the dupes sometimes will get stuck on a ghost task: the item does not get dropped, energy is still consumed (if manual generator, energy is still produced), no additional items are consumed, and animations play from the beginning.
I've no idea what it might be, it's also really close to the ghost progress bar bug, a minor visual bug where the progress bar disappears, usually after changing layers.
Those bugs didn't happend back when I started playing the same last year, but after some days, my game has been on this state and it's almost one year.
It affects ALL games, modded, unmodded, 1x or 3x speed, DLC or not.
