Entombed duplicants lack their heads. This is normally never a problem as most blocks are opaque, but transparent blocks (i.e Glass) tend to reveal that the animation lacks a head.
Steps to Reproduce
Entomb any duplicant in any glass or another type of transparent tile. This is best done in Sandbox mode.
