Old bug. It's been happening since back when Glhrmzz and Flare2V streamed the game regularly. Thought I'd report it again since the bug tracker's reopened.

Steps to Reproduce
Go in and out of insanity, either by using sanity raising/lowering items, or simply fighting shadows.
Go to bugs

Yeah, these things happens to me, too, but only when I play for hours (usually with mods).

Frames go back to normal when you exit to the menu and loading the world again.
