This will occur when you play with controller mode.
The Tar Lamp can be equipped not only in the hand, but also on the boat. This means you can equip two Tar Lamps at the same time on the HUD.
But "Unequip" action via D-Pad doesn't distinguish the different of them. If you unequip the one in the hand, the one on the boat is unequipped too, and vanish.

Steps to Reproduce
1. Ride on the boat.
2. Equip a Tar Lamp in the hand.
3. Equip another Tar Lamp on the boat.
4. Unequip the Tar Lamp in the hand via D-Pad Right.
Why? Tar Lamp on the boat is gone!
