Shadow Maul drains Wanda's sanity, assuming she never has full health. This item doesn't restore Wanda's health, but it does drain sanity using the mechanics above. Technically, Wanda doesn't have a health bar, but rather an age bar.

Please fix the Shadow Maul code specifically for Wanda so she doesn't suffer the insurmountable penalty of an absurd sanity drain in exchange for absolutely nothing (she won't regain "health" with this weapon).
