It seems that once I have become Wonkey, not only the Compendium does not unlock Wonkey in Survivors tab (which means I cannot change skins in there), the game does not record anything I do while in Wonkey form. This includes Scrapbook does not record the quotes when I examine new items, and my death is not recorded in Obituaries. Once I return to Human form, everything suddenly works again.
Steps to Reproduce
Simply become Wonkey
