Fail to revive by all means

I am playing on a dedicated server as Wolfgang and I am experiencing a bug where my character cannot revive either by telltale, amulet, touchstone or the portal. I noticed when I'm a ghost for more than maybe 5 mins, the screen goes red/black and white and the map washed out. Furthermore, I noticed the status mod for body temperature is gone too. Usually, it is stated at a constant of 35* when you're a ghost. My pet is left wondering by itself too, it stopped following me. I can't haunt or do anything. The first time when this happens the admin had to restart my character back but it took a lot of tries. Even the c_despawn didn't work. Could you please fix this issue. Thank you.
