Wendy creating a new grave will cause her screen to go black and break her movement

I'm unable to reproduce the issue. Could you try reproducing the issue with no mods enabled? Including client-side mods?

The Geometric Placement mod has shown similar issues before.

Funnily you were right, I had disabled all server mods but not client ones. Once I disabled geometric placement it worked as intended, thank you DiogoW!

Nooo I had marvel rivals open in the back because I had to stop playing due to the bug lmao 

Since this is still happening and in case someone makes it here after frantic searching like I did, you don't actually have to remove Geometric Placement to prevent the bug. Holding ctrl (or whatever key you have bound to temporarily disable the grid) while creating the grave will let you place it just fine.
