Simple as it sounds, taking damage while transferring consciousness during the black screen flash will softlock you. You cannot open the pause menu, pause the game, or do anything except alt+f4.
Steps to Reproduce
1) Attempt to transfer consciousness while in danger (this is easiest to time using the map transfer skill)
2) Take damage during the "shut-off" black screen
3) Observe your glorious black screen for all eternity as WX's consciousness is erased into the aether
A developer has marked this issue as fixed. This means that the issue has been addressed in the current development build and will likely be in the next update.
