Just reproduced it three times in a row. Playing on Windows 7, playing as Wendy (if that helps).
I normally don't play Wendy, and I've never had this happen before so not sure if that's an issue or what. I'll disable all my mods and update this if that makes a difference.
FYI: disabled all mods and reproduced it a fourth time.
