When someone attunes to it the last person to be at tuned should get their 40 health back, multiple people can be attuned to the same one, or have it work like the Telltale heart and known attunes to it and anybody can use it by possessing it (also if you go with option two you don’t have to, but I prefer if when one of the people are attuned to it, uses it anyone else that was attuned to it gets their 40 health back)
