While playing splitscreen offline, I was running away from a dog attack while carrying a backpack with my tools, in the end they killed me and I resuscitated via a meat effigy. I returned to the location to pick up my stuff and all the items in my bag were bugged, none of them was showing the percentage of usage and when trying to equip them the character dropped them instead, after picking them up they went back to normal.
Unfortunately I didn't took a snapshot, but will try to replicate in a while
