I did the Cyclum puzzle twice, around the time it initially came out, and after I finished the Metheus puzzle, to my surprise I was awarded the tragic torch skin a second time. Additionally, if you craft a torch in-game, the tragic torch option will appear twice, which is kind of annoying.
Steps to Reproduce
If you already got the tragic torch skin my doing the Cyclum puzzle around the time it came out, doing it a second time may award you the tragic torch skin again.
